Pain is not well controlled

The patient has frequent discomfort, changes in pain levels or difficulty resting, moving or maintaining a basic daily routine at home.

Digestive symptoms are affecting daily life

Nausea, vomiting, changes in bowel habits or difficulty eating properly are causing concern and affecting the patient's quality of life.

The patient is losing weight or appetite

Eating is becoming increasingly difficult, with changes in taste, aversion to certain foods or concern about weight loss during treatment.

Sleep, mobility or independence have changed

The patient is sleeping worse, moving less, feeling weaker or needing more help with activities they were previously able to manage more independently.

How we start

First we understand the situation. Then we define the next steps.

We do not ask the family to decide on their own what the patient needs. We begin by understanding the clinical situation, the main symptoms and the impact treatment is having at home.

We listen to the patient's situation, current treatment, main symptoms and the family's concerns.

We carry out an initial assessment

We analyse the medical, functional and family situation to identify priorities, risks and real needs at home.

We define a care plan

If the patient needs ongoing support, we propose a follow-up tailored to their clinical situation and clearly explain the next steps.

If after the assessment we find that you do not need clinical follow-up, we will tell you that honestly.

Lo importante durante el proceso oncológico en casa

¿Humana sustituye al oncólogo?

No. Humana no sustituye al oncólogo ni al equipo hospitalario. Acompañamos al paciente en casa, ayudamos a controlar síntomas y coordinamos el seguimiento para que la familia tenga una referencia clínica clara.

¿Puede ayudar durante la quimioterapia?

Sí. Podemos ayudar a hacer seguimiento de síntomas frecuentes durante el tratamiento, como náuseas, cansancio, pérdida de apetito, dolor, alteraciones del sueño o cambios en la movilidad.

¿Puede ayudar con las náuseas, los vómitos o los problemas digestivos?

Sí. Durante el tratamiento oncológico son frecuentes las náuseas, los vómitos, la pérdida de apetito o las alteraciones del ritmo intestinal. El seguimiento clínico ayuda a valorar estos síntomas, anticipar problemas y orientar a la familia sobre los pasos adecuados en cada situación.

¿Puede ayudar después del tratamiento?

Sí. Después del tratamiento también pueden aparecer fatiga, debilidad, problemas de alimentación, cambios emocionales o dudas sobre la recuperación. El seguimiento ayuda a ordenar esa etapa.

¿Ayudáis con el control del dolor?

Sí. El control del dolor es una parte importante del seguimiento. Valoramos la situación, revisamos la evolución y orientamos a la familia sobre los pasos adecuados según el caso.

¿Incluye apoyo en alimentación?

Sí. Podemos orientar sobre alimentación, hidratación, pérdida de apetito, pérdida de peso y dificultades que aparecen durante el tratamiento o en la recuperación posterior.

¿Puede incluir fisioterapia o apoyo a la movilidad?

Sí. Cuando hay debilidad, pérdida de movilidad o dificultad para mantener autonomía, podemos valorar la necesidad de fisioterapia u otros apoyos profesionales.

¿Puede ayudar aunque ya tengamos cuidador?

Sí. El cuidador puede ser fundamental en el día a día. Humana aporta criterio clínico, seguimiento y coordinación para que la familia no tenga que tomar sola todas las decisiones.

¿La valoración inicial obliga a contratar seguimiento?

No. La valoración permite entender la situación y recibir una orientación profesional. Después, la familia decide libremente cómo quiere continuar.

¿Necesitas apoyo durante el proceso oncológico en casa?
