If the patient requires continuity of care, we propose follow-up adapted to their evolution and clearly explain the next steps.

If, after the assessment, we believe clinical follow-up is not necessary, we will tell you honestly.

What matters when care continues over time

When does it make sense to ask for help for a chronic patient?

When the family feels that care is becoming increasingly difficult to sustain: more symptoms, more doubts, more appointments or more decisions to make at home.

Is it useful even if the patient already has specialists?

Yes. Humana does not replace specialists. It helps organise information, support the patient’s evolution and better coordinate day-to-day decisions.

Can it help if the disease is gradually worsening?

Yes. In many chronic conditions, changes appear progressively. Detecting them early helps adjust care before the situation becomes more complicated.

Can it help even if we already have a caregiver?

Yes. For us, the caregiver — whether family or professional — is the first level of healthcare. Humana does not come to replace them, but to support them. We provide clinical direction, follow-up and coordination so they can act with greater confidence and do not have to make important decisions blindly.

What happens if new needs appear?

If the patient changes, worsens or needs more support, we review the situation and guide the family on the most appropriate next steps.

What should we do if a medical emergency appears?

Our goal is to anticipate problems before they become an emergency. Even so, if a life-threatening situation appears, emergency services must be contacted immediately. In urgent but non-life-threatening situations, we guide the family in activating the available resources, such as the patient’s private health insurance. Our role is to help coordinate care and ensure that the subsequent follow-up is safe and organised.

Does the initial assessment require follow-up care?

No. The assessment helps understand the situation and provides professional guidance. Afterwards, the family is free to decide how they wish to continue.
