The initial home-based clinical assessment allows us to understand the patient’s medical, functional and family situation before recommending any type of follow-up.

We do not ask the family to choose a fixed plan. First, we organise the case, identify risks and define what type of support makes sense.

What matters before getting started

Does the initial assessment require me to book follow-up care?

No. The assessment helps us understand the clinical situation and provide a professional recommendation. Afterwards, the family freely decides how they want to continue.

Why don’t you publish fixed plans?

Because each patient may need a different combination of medicine, nursing, physiotherapy and other specialised support. Publishing fixed packages can oversimplify cases that are clinically different.

What happens after the assessment?

The Humana team organises the case information and proposes the next steps. If the patient needs continuity of care, a personalised follow-up proposal is prepared.

How much does it cost to get started?

The initial home-based clinical assessment costs €150.

How is the follow-up proposal calculated?

It depends on the medical situation, the patient’s autonomy, the clinical risk, the professionals required and the level of coordination needed by the family environment.

Can it include physiotherapy?

Yes. In many cases, physiotherapy can be a central part of follow-up, especially after a stroke, hip surgery, loss of mobility or a functional recovery process.

Can it include nutrition, psychology or other support?

Yes. If the case requires it, the proposal can include other specialised support within a coordinated clinical approach.

Does Humana replace the caregiver?

No. The caregiver remains a key part of the patient’s day-to-day life. Humana provides clinical direction, follow-up and coordination so that the family does not have to make every decision alone.

Can the follow-up change over time?

Yes. If the patient improves, worsens or new needs appear, Humana can review the proposal and suggest adjustments transparently.
